Mechanism of Action
Semax acts through melanocortin receptors and modulates monoamine neurotransmitter systems, but its most clinically significant mechanism appears to be upregulation of BDNF (brain-derived neurotrophic factor) and NGF (nerve growth factor) — proteins essential for neuronal survival, plasticity, and learning.

BDNF Upregulation
BDNF increases persist for hours after administration despite the peptide’s short plasma half-life, suggesting receptor-mediated gene expression changes rather than simple agonism. This mechanism explains the sustained cognitive effects reported in clinical and preclinical studies.

Neuroprotection
In stroke models, Semax reduces ischaemic damage and promotes recovery. Russian clinical guidelines include it as an adjunctive treatment in ischaemic stroke, representing one of the few formally approved applications for a nootropic peptide.
