Cagrilintide
Mechanism of Action
Cagrilintide binds amylin receptors (AMY1-3) in the hypothalamus and area postrema, reducing appetite through a complementary pathway to GLP-1 agonism — acting on calcitonin receptor complexes that mediate satiety and slow gastric emptying.
Complementary to GLP-1
Amylin and GLP-1 receptor systems act through partially distinct hypothalamic circuits. Dual activation with cagrilintide + semaglutide (CagriSema) produces additive weight loss beyond either agent alone — the rationale for combining them in Phase III trials.
REDEFINE Phase III
The REDEFINE-1 trial of once-weekly CagriSema (2.4 mg sema + 2.4 mg cagrilintide) demonstrated 22.7% mean weight reduction at 68 weeks, with over 40% of participants achieving ≥25% weight loss.
